This is a dental discount plan and is not dental insurance. It cannot be combined with any other dental insurance.
The plan is good only at Harbor Breeze Dental Care. If you are referred to a specialist, they will not offer this discount.
If a patient is referred to a specialist and found to have periodontal disease, the two included hygiene visits will cover periodontal maintenance (D4910) only after deep scalings have been performed and cleared by a periodontist.
The plan cannot be used for treatment following an injury where a lawsuit, outside medical, disability or workers' compensation insurance is involved.
The plan is non-transferable. Family members cannot be substituted for another family member.
The plan is non-refundable. No refunds are given if the plan is not used.
Rates are subject to change annually.
Payment for services is due at the time of service.
This offer cannot be combined with any other offers.
For orthodontic treatment, the participant must remain a plan member for the entire duration of treatment.
Dental services only; products are not included.
